How to Fill Out Civil Service Form 100 (Revised September 2016)

Civil Service Form 100 is the official application form used when applying for the Civil Service Examination in the Philippines. The latest version used by the Civil Service Commission is Form No. 100 Revised September 2016. It must be properly filled out and submitted with your requirements when filing for the CSE-PPT (Pen and Paper Test).

Use black ink and print legibly. Do not leave any required field blank. Here's a section-by-section guide:

PART I – Personal Information

• Name, Birthdate, Sex, Status: Match exactly what appears on your valid ID.

• Place of Birth, Citizenship: Ensure you are eligible (must be Filipino).

• Address & Contact Info: Include house number, street, barangay, city, and mobile number.

PART II – Examination Details

• Level: Choose either Professional or Sub-Professional.

• Date & Place of Examination: Leave blank if unknown—CSC will fill in.

• Preferred Test Center: Based on your region.

PART III – Educational Background

• Write the highest educational attainment, including:

  • School name

  • Course

  • Year graduated

PART IV – Signature & Thumbprint

• Sign and affix your left and right thumbmarks in front of CSC personnel only.

• Do not sign ahead of time.

• Print it on long bond paper (8.5" x 13").

• Attach 4 recent passport-sized photos with name tag (full name, signature, taken within 3 months).

• Bring your valid ID (original and photocopy).

• Double-check for spelling errors or blank fields.

Submit the accomplished form at the nearest CSC Regional or Field Office together with:

• Valid ID

• Passport photos

• ₱500 exam fee

Walk-in submissions are first come, first served during the application period.

• Application starts: May 12, 2025 - June 11, 2025

• Exam date: August 10, 2025
